Care Card Claim

FWD Care is the privileged benefit for the insured to have medical treatment at FWD registered hospitals. The holders of FWD Cards are able to obtain treatment without paying in advance (excluding costs that exceed the benefits of your coverage). However, certain conditions apply.

This special services provide to our insured for convenience in medical service in order to good health.

If the insured need to receive medical service due to illness or injury as inpatient or outpatient in hospital of FWD Care Network, the insured can use FWD Care under the conditions.



How to use FWD Care Card

If you receive treatments from one of our network hospitals, then the company will pay directly to the hospital on your behalf according to the terms of your plan. Just show your card.

Out-Patient

  1. Present FWD Card (check expiry date) along with your ID card or student card or a valid documents issued by the government. Then fill in the form (Outpatient “Health Care” form) provided by the hospital in part of the insured section with your signature.
  2. Upon discharge, hospital officer will review your total expenditures to ensure that it meets the agreed coverage benefits and conditions stated in the insurance policy. For the amount that is covered by the company, the hospital directly charges the company and the customer do not pay any amount within the coverage. If the total costs exceed the coverage, the hospital will charge the excess amount directly to the policy owner.

Note:

  1. Upon using this cashless service, FWD Card must be presented by the policy owner every time he/she visits the hospital or else the policy owner must pay in advance and submit documents to the company (Claims by normal process).
  2. The company shall assess accidental claim after we receive billing from the hospitals where the insured have medical treatment using FWD card.
  3. Hospitals normally submit billing to the company for reimbursement at the end of the month or the end of next month therefore if the insured person needs the company to pay accidental claim (AI) earlier than the normal cycle, insured person has to submit claim form together with medical certificate directly to the company.

In-patient

  1. Present FWD Card (check expiry date) along with your ID card or student card or a valid document issued by the government. Then fill in the form (Inpatient “Health Care” form) provided by the hospital in part 1 of the policy owner section with your signature.
  2. After visiting the doctor, if you need to be admitted in the hospital for further treatments, the hospital officer will contact the Claims Department to clarify and request a permission to use FWD Card. If accepted, you will be treated without payments (conditions may apply). However, if your sickness is serious such as accident or emergency case, you will be treated immediately without needing to seek permission from the Claims Department. After the treatment, the hospital will then contact the Claims Department to process the claims.
  3. Upon discharge, the hospital will inform the Claims Department about the total admission cost. The cost must be under the agreed coverage benefits and conditions stated in the insurance policy upon purchase. If the total costs exceed the coverage, the hospital will charge the excess amount directly to the insured person.
  4. If you have purchase other policies such as daily hospital cash benefit (HB) or Accidental Indemnity (AI), we will provide benefits as agreed in the policy automatically to you upon discharge. There is no need to submit documents to the Claims Department except in special cases where we will inform you.

Note:

  1. If the company detects cause of illness or injury is the exclusion conditions of the policy, the company can reject this claim although the company approved the claim.
  2. If you have paid medical expenses for hospitalization and not use FWD Card at the network hospital, you can submit documents to the Claims Department to reimburse your payments.
