How to file a claim

Required documents

  • A certified true copy of death certificate – certified by the beneficiary
  • A certified true copy of the insured’s ID card – certified by the beneficiary
  • A certified true copy of the insured’s census with a “death” stamp from a local registrar - certified by the beneficiary
  • A certified true copy of the beneficiary’s ID card
  • A certified true copy of the beneficiary’s census
  • A photo of the beneficiary's face with ID card
  • FATCA declaration form
  • Tax residency self-certification form
  • In case of death from non-natural causes (e.g. accidental death, homicide, suicide, death by animal, undetermined): Report from Royal Thai Police/the autopsy report – certified by a police officer or the pathologist
