1. Give us a call on 1351 to check how much you owe (both your outstanding unpaid premiums along with any interest).
2. Make a bank payment, either internet banking
3. Prepare documents
   • Policy changes and renewal request form
   • A certified true copy of you ID card
4. Send documents to us
   • Submit online documents, click (Please choose topic: Change policy details > Reinstate policy)
   • Through your insurance agent or in person via FWD head office or any branch offices
   • Mail all required documents to
     Policy owner service department, FWD Life Insurance Public Company Limited, No. 6 O-NES Tower, 4th, 22nd-23rd Floors Soi Sukhumvit 6, Khlong Toei Sub-District, Khlong Toei District, Bangkok 10110

We’ll then give you any updates on your policy reinstatement via SMS.
