Customers born in May: Eligible period on 1 April - 30 June 2024

Customers born in June: Eligible period on 1 May - 31 July 2024

Mr. F. was born in April. He purchases a new policy in April 2024 with annual premium payment of 20,000 baht through a life insurance agent and the policy approved in April 2024. He will receive Lazada E-Coupon worth 600 Baht via SMS within June 2024.

Terms & Conditions

• For current FWD Life Insurance customers born in April - June whose name is the ordinary policy payer, with the effective policy status and the premium has been paid continuously in 2024, only for policies purchased through life insurance agent, affiliate, and online channels.
• Eligible customers for Lazada E-Coupon are those purchasing new policy with FWD Life Insurance within within campaign period via life insurance agent, affiliate, and online channels and in campaign listed, except via Siam Commercial Bank Public Company Limited. Moreover, the customer shall be the premium payer or the insured in the new policy which must be valid until the date of receiving the reward.

• Unit Linked, Group Insurance, Credit life, MRTA, SME and PA policies are not eligible for this promotion.

• Lazada E-Coupon will be delivered via text message (SMS) to the phone number provided by the customer to FWD Life Insurance, when the new policy payment is completed, approved and has been in effect for 45 days.

• Reserves the right to receive Lazada E-Coupon of 1 prize per 1 policy, purchase more than one win more prize according to policies owned and its value when paying the first year’s premium as follows:
  Annual premium payment term and single premium payment
  (1) Lazada E-Coupon valued at 600 baht for premium payment between 20,000 and 99,999 baht per policy
  (2) Lazada E-Coupon valued at 3,000 baht for premium payment of 100,000 baht or more.
  Other premium payment terms: monthly, 3 months, 6 months
  (1) Lazada E-Coupon valued at 300 baht for premium payment between 20,000 and 99,999 baht per policy
  (2) Lazada E-Coupon valued at 1,500 baht for premium payment of 100,000 baht or more.

• The insurance premium calculation depends on the type of insurance and payment condition. In case of ordinary policy with regular premium payment, 100% of the premium shall be calculated. In case of the single premium, 10% of the premium payment shall be calculated. This however does not include Unit Linked.
• Lazada E-Coupon cannot be exchanged or reimbursed for cash or other benefits and cannot be used as a discount for premium payments.
• FWD Life Insurance reserves the right to change any prizes with equal or similar value without prior notice.
• FWD Life Insurance is not involved in distributing the prizes listed in this promotion. The company is not responsible for any quality or service that may occur due to such rewards.
